On the left below is an empty registration form, and on the right is an example of a filled form. Uploading a picture of a registration form or a document with similar info is mandatory to receive Plastic Credit funding. Hence, in the "registration form" field you can upload either;
Picture of filled Empower registration form template, WITH SIGNATURE
Picture of similar info from your own inventory systems, WITH SIGNATURE
Minimum: picture of note with total volume and estimated contributors, WITH SIGNATURE
Note that the registration form shown includes the following:
names of all individuals who participated in the collection/clean-up with the amount they each collected,
how much they were each paid
a signature from a staff member of the organization.
This is the gold standard, but if you are able to upload similar information in other shapes, this is also acceptable.
